Carrie Hulett was employed at the the office of David Balfour, O.D., Bushnell, McDonough County, Illinois from July of 2003 to August of 2007. She was fired and subsequently charged with theft.

In June of 2008 she was arrested by the Illinois State Police and charged with three felonies, theft of cash, Identy theft, and Credit card theft of over $10,000. Here is a link to an article published in the Peoria Journal Star in 2008. This case was resolved and I was notified by this letter. Dear Dr. Balfour:

We are writing to advise you that the above-named defendant was sentenced in McDonough

County Circuit Court on June 5, 2009 for the offense of theft.

The court, after considering the nature of the offense, imposed the following sentence:

  • Thirty Months Probation. The defendant will also provide a DNA sample the to McDonough County Sheriff's office and will be responsible for the $200.00 fee.
  • Serve 90 days in the McDonough Jail as follows: serve 3 consecutive weekends with the balance of 84 days stayed until March 10, 2010
  • Pay restitution in the amount of $37,094.87 for the benefit of David Balfour. Pay court costs and penalties. The defendant will also enter into a payment agreement with the State's Attorney's Collection Specialist.
  • Perform 150 hours of community service

  • Not violate any criminal statue of any jurisdiction
